| 2) | Abstracts should be written based on the style of CLINICAL NEUROPHYSIOLOGY. They should not exceed 300 words. Abstracts should adhere to the following format: Objective, Methods, Results, Conclusions. |
| 3) | Abstracts by speakers of named lectures, special lectures and symposium/workshop speakers are also limited to 300 words but need not follow the above style. |
| 2. | The submission can be accepted only via online system on the UMIN web site. UMIN (University hospital Medical INformation Network) is a cooperative organization for national medical schools in Japan, sponsored by the Ministry of Education, Culsutre, Science, Sports and Technology (MEXT), Japan. |

| 3) | All information on the registered abstract will be sent to your registered e-mail address. This confirmation mail will include the abstract registration number. |
| 4) | Up to 6 facilities can be registered by default as Affiliated Institutions. By clicking the input column button, up to maximum of 10 facilities can be registered. |
| 5) | Up to 6 persons can be registered by default as Co-Authors. By clicking the input column button, up to maximum of 20 persons can be registered. |
| 6) | You can input up to three Categories. Category(1st Choice) and Category(2nd Choice) will be the required items. |
| 7) | The title of abstract should be no more than 150 characters. |
| 8a) | The abstract body should be no more than 300 words. |
| 8b) | Abstracts should not include any diagrams, charts or reference citations. |
| 8c) | Please paste text processed off-line into the designated box on the Submission Form. |
| 8d) | Please pay careful attention to follow the instructions on the Submission Form regarding use of specific character symbols, Greek letters, superscript, subscript, italic, bold or underlined text. You may copy the code on the Submission Form and paste (insert) it as appropriate. |

| 10) | If you would like to apply for the Young Investigator Award, please check the box of "Application for Young Investigator Award". | 11) | Password is case sensitive and must be between 6 and 8 characters. This password is necessary for you to view or edit/revise your abstract. Please make sure to keep notes of your password. For the protection of personal information we can NOT deal with any inquiries regarding your lost/ forgotten password. |
| 12) | When you click the "confirm to submit" button, a confirmation page will appear.Note than your submission will not be completed until you click the "Register"button. |

A confirmation e-mail will be sent to your registered e-mail address after your submission is confirmed. If you do not receive the confirmation mail, your abstract may not have been correctly registered or your e-mail address may be wrong.
Please confirm your registered e-mail address on the confirmation and correction page.
For security reasons, we recommend you not to use plaintext-based web communications for your abstract submissions if possible.
